I hereby provide Mr. Petar Vack - Accordion School Dobřany, ID: 8320721, Tax ID: CZ8903302100 address Zahradní 1335, 33441 Dobřany (hereinafter referred to as the "organizer") as the organizer of the Accordion Competition - 19 - 20 June 2021, my personal data and other data indicated in the application, and hereby grants him his express consent in accordance with the provisions of § 5 of Act No. 101/2000 Coll., on the protection of personal data and on the amendment of certain laws, as amended, to the processing of personal data and other data provided by the user.

I, as a participant in the competition, provide the organizer with personal data in the scope of name, surname, e-mail address, gender, and other data, for the purpose of contacting and sending business communications in the sense of Act No. 480/2004 Coll..

I also express my consent to the processing of the photos and film recordings taken for marketing purposes, especially the publication on the website www.accordionschooldobrany.com and the Facebook account. Furthermore, I agree to the publication of the results of the competition, including the full name and the result expressed in points and a verbal evaluation, on the website www.accordionschooldobrany.com. The organizer guarantees me, as a participant, the other rights listed in § 11 and 21 of Act No. 101/2000 Coll., on the protection of personal data and on the amendment of certain laws, as amended, i.e. the right to access information on the processing of the participant's personal data, and the participant's right to ask the organizer for an explanation and removal of an undesirable situation, if he believes that the organizer is processing the participant's personal data in violation of the protection of private and personal life or in violation of the law, especially if the personal data is inaccurate with regard to the purpose of the processing. For participants under the age of 16, this consent is given by their legal representatives.
